چکیده

We report a case of 73-year-old man who developed nonconvulsive status epilepticus as complication cerebral hyperperfusion syndrome after carotid endarterectomy for artery stenosis. On postoperative day 1, the patient experienced headaches and vomiting. Resting N-isopropyl-p-[123I] iodoamphetamine single-photon emission computed tomography showed increased blood flow to entire right hemisphere, was diagnosed with syndrome. He treated antihypertensive antiseizure medications, sedated using propofol, intubated, placed under mechanical ventilation. 3, perfusion imaging reduction in hyperperfusion, propofol sedation terminated on 4. However, exhibited prolonged impaired awareness roving eye movements, long-term video electroencephalographic monitoring revealed electrographic seizures. The epilepticus. Propofol resumed, medication dose increased. Subsequently, state hemisphere diminished, findings improved, allowing be 7. from this suggest that when clinical subtle symptoms, such are observed during treatment syndrome, electroencephalography should performed detect

Commentary Cerebral hyperperfusion syndrome following carotid endarterectomy

Extracranial internal carotid artery stenosis accounts for 15–20% of ischaemic strokes and carotid endarterectomy (CEA) is the most frequently performed surgical intervention in stroke prevention. The risk of stroke and death associated with the operation has been estimated at about 5.6% (95%CI 4.4–6.9). Neurological complications following CEA are usually ischaemic in nature, due to embolizati...

Cerebral hyperperfusion syndrome occurring three weeks after carotid endarterectomy.

Cerebral hyperperfusion syndrome is a recognized complication of carotid endarterectomy. Various studies have documented an incidence of 0.3 to 1.2%. It occurs in the setting of sudden reperfusion of a chronically hypoperfused hemisphere. We present here a case of a 48-year-old lady who developed cerebral hyperperfusion syndrome three weeks after undergoing a carotid endarterectomy for high-gra...

Delayed Cerebral Hyperperfusion Syndrome Three Weeks after Carotid Artery Stenting Presenting as Status Epilepticus

Cerebral hyperperfusion syndrome (CHS) is increasingly recognized as an uncommon, but serious, complication subsequent to carotid artery stenting (CAS) and carotid endarterectomy (CEA). The onset of CHS generally occurs within two weeks of CEA and CAS, and a delay in the onset of CHS of over one week after CAS is quite rare. We describe a patient who developed CHS three weeks after CAS with sta...

Nonconvulsive status epilepticus manifesting as bradyphrenia: a case report

Generalised convulsive status epilepticus continues to be a medical emergency with high morbidity and mortality. The patient with convulsive status epilepticus has continuous or rapidly repeating seizures. In contrast, symptoms in nonconvulsive status epilepticus are often more subtle which frequently delays the diagnosis. This case describes a 27 year-old man who presented after a first seizur...

Postoperative Cortical Neural Loss Associated With Cerebral Hyperperfusion and Cognitive Impairment After Carotid Endarterectomy

Background and Purpose—Although cerebral hyperperfusion after carotid endarterectomy (CEA) often impairs cognitive function, MRI does not always demonstrate structural brain damage associated with postoperative cognitive impairment. The purpose of the present study was to determine whether postoperative cortical neural loss, which can be detected by I-iomazenil single-photon emission CT, is ass...
